Mine just floats in front of my face... that way I don't get any shake from my hands, and it makes panning soooo much easier. You have to have special powers like me though, in order to get something to float in front of you like that.

OK, so my camera doesn't actually float, but I do have magical powers

. I stole them from Bob!
I hold mine with my left hand under the lens when hand-holding, and I'm going to try the hand over the lens with the tripod. I keep my elbows in against my sides, unless I have something to brace them on. I truly do the whole let-your-breath-out-and-hold-it thing too. I don't think about it, but if I do a series of continuous photos I notice because I have to inhale when I'm done. Just like shooting..., exhale, hold, squeeze, follow through. My favorite thing to watch with photographers is our smashed noses.

You'd think in today's world of technology, they could design a camera with enough relief that the viewfinder would be slightly out so your nose wasn't flat and crooked after a wedding!
