About the fake head - the shiny skin isn't too much of a problem, you are looking for where the shadows hit.
The eyes are an issue.  Someone, maybe on this forum or in a book, said they got around this problem by using a permanent marker to color in the fake catchlights and used clear finger nail polish to paint shine onto the eyes so they could see the real catchlights.

Mike's diagram is very good.  It's actually similar to what I do. 
BTW, none of my stuff up lately has more than just a main and a fill light, as you know I don't have a hair light.  Or didn't - I got my brand new lights in the mail the other day!!!!!!  So once I get a stand then I will have a hair light.

Thanks, Benji! I tried your way first. I have never heard of having the fill on the same side as the main.

See the two images below.  The only thing I moved between them was the fill light. Note the darker shadows in the one where the main and fill are both on the same side. 

To clarify, the fill is on the same side as the main light but still behind the camera but only slightly off to the main light side.

This is how I do my lights.  The little circle with the funny shoulders is my person, the half circles are soft boxes, although for the fill I have started using a white umbrella.
Keep in mind that I put my fill quite high, it tends to wrap around them and light the background nicely too.  Of course, I have developed this specifically to what I am shooting and the look I want.  I don't use backgrounds, but bedroom-type sets.  You have to experiment.

Oh and sometimes I switch my meter readings and make the center my main and fill one side with the side light (I don't physically move the lights) with a low ratio and I get a modified butterfly lighting, it's not a real pattern, you won't find in a book, but I am loving the result.

As Benji posted, the fill on the opposite side will do the same as though it is on the same side if moved back. There is no control of the shadow side if the lights are on the one side, where's the fill ratio.?

The one on the left is flat lighting, both lights at same distance and intensity.
The one on the right, the lights are on the same side, thus the shadow side is not filled, the result is no ratio for the shadow side.

Mike, having the fill centered and to the same side but so far back sorta wraps around the whole subject and fills in the shadows.  Because of the wrapping I can even stand in front of it a bit with no effect.

Ok, I don't have any examples where I am at, except this one.  I posted it in another thread last night, so forgive the repetition.
Lights are both on the same side.

Wrap, no comment.

No arguments, just a discussion, but, I would try to rephrase the wrap of light on a subject with a fill light, no can do.   Big Grin

If you think light wraps around the subject, then answer these statements.

"There's no wrapping of light when backlighting a subject."
"A kicker light does not wrap around a subject"

Posted is a modified Butterfly light with a fill on the right. main lighting/butterfly is slightly to camera right.

Subject has light "FALL OFF" on her right. there is NO FILL lighting on her right.

Mike..... half of what you said is not translating into a logical meaning in my head... I think it may be on my side that the miscommunication is occuring.

And right, light doesn't really "wrap" per se...  science says it just goes one direction and stops if something is in it's way... ok, well it doesn't "stop", it's absorbed or bounced back the direction it came.  And it really doesn't "bounce" it's reflected, and the amount that is reflected depends upon the surface it is hitting.  In fact, part of the light can be absorbed and part of the light can be reflected off the same surface!  How 'bout that?

In conclusion, apparently I am setting up my lights incorrectly.  However, I do like the results, but it's possible I might change my mind, so, well - no comment, except that there is more than one way to skin a cat... I think, never tried it.

there is NO FILL lighting on her right.

But it's not black, some light is reaching it.

And, btw, I am not arguing either Smiley  Actually I learn more about photography in these sort of conversations than anything else.

You know best, thanks for the insight.
A small room, I can see the reflected light.

In a studio with just two lights, the shadow would be more pronounced, however, in this case, as you mentioned, reflected light.

I'm used to a large room.
